No problem. Arrive 15 minutes before your appointment and we'll hand you a clipboard. Either way, you'll be seen on time.
What we'll ask
- Your contact information and emergency contact
- Medical history and any current medications
- Insurance card information (if you have one)
- Your dental concerns and goals — what brought you in
- How you'd prefer we contact you (call, text, or email)
